Social Service Alignment Learning Collaborative

The Comprehensive Care, Community and Culture Program (C4P) program built on the established University of Chicago’s Comprehensive Care Program (CCP) is based on the premise that health and well-being are influenced by both medical care and by social and behavioral processes that are strongly affected by the social context within which people live. Accordingly, interventions to improve health must address both medical and social determinants of health, and aligning such efforts is critical. The goal of this program is to develop effective and affordable strategies to align medical and social services to improve health outcomes and well-being of individuals and promote health equity for disadvantaged populations and communities. Our curriculum brings together leaders and rising leaders from local social service organizations and government working in the housing, public benefits, transportation and other key sectors that are highly prevalent unmet needs in this group. The goal will be to enhance cross-organizational collaboration and develop targeted, innovative solutions to better align systems to meet basic social needs through case based learning.
